Winter sport physical packets and recertification forms are posted on the Swimming & Diving team website (below). If a student is not currently competing in a fall sport, the full physical packet must be completed. If a student is already in a fall sport, only the recertification form needs to be completed.
Families have the option of having a physical completed by their personal doctor.
If interested in attaining a sports physical on Monday, November 13 at Bishop McDevitt (and /or participating in a free EKG Heart Screen opportunity), please click on the link below and sign up for a timeslot. Physicals and screenings will be offered from 8-11 a.m. Please choose one selection per person and fill out the appropriate paperwork linked below.
The Sports Physicals are $15: checks are preferred, but cash will be accepted.

All forms must be completed before the allotted time frame. If you are not interested in the EKG Screening, Rochelle Blakely does not need a parental consent form.
